HOW TO PRINT A PLAYER REGISTRATION FORM

• Click on the Registration tab on the home page of our website
  
• Select “General Registration”  (towards the bottom of the page)
• Log-in as an “Existing Player” (The e-mail address you originally registered with and password will be required)
• Click on your Player's Name in the Sub-Account(s) box (Lower middle of this page)
• Click on the FORMS button (on the left side of the resulting screen)
• Click on the Player Registration Form next to AYSO Fall (Current Year)  (on the right hand side of the screen) to download the Registration Form in PDF format.
• Print the form
• Check the form for accuracy
• If necessary update your contact information (Click here for directions on how to do this)
• Repeat above steps if you make any changes.
  
• Sign the form and give it to your Coach or Team Manager
